The Coalition of Community Health Clinics (CCHC) is a non-profit organization that serves a network of seventeen non-profit community health clinics that provide a range of health and preventive services for low-income, uninsured, and under-served community members in Multnomah, Clackamas, and Washington Counties. CCHC’s mission is to improve healthcare for vulnerable populations residing in Tri- County area by strengthening community clinics, promoting health equity, and fostering collaboration through shared learning and advocacy efforts with our members. CCHC’s programs include: '''OHP Enrollment''' To schedule an appointment for OHP Enrollment help or to get information updated call CCHC Health Navigator & Enrollment Specialist at ( ''503) 546-'' ''4991'' or write to their community health worker team at ''chw@coalition-clinics.org'' . CCHC partners with CareOregon. Connect to Care’s OHP Enrollment assistant at CareOregon Customer Service at 503-416-4100. CCHC is involved in a partnership with ''JOIN’s'' six community agencies who are working together to support community members transitioning from homelessness to stable housing. The '''Mobile Permanent Supportive Housing (MPSH)''' retention team provides wrap around services including case management, mental health, health advocacy and peer support services to individuals and families new to housing.
