Blackburn Center offers compassionate care for physical and mental health all in one setting. Their mental health services include counseling, medication management, individual and group therapy as well as specialist referral if needed. This clinic can also provide addiction/substance use treatment/recovery services.

Since 2015, Imani Center has provided mental health and addiction treatment for the Black and African American community. Imani Center offers a culturally safe environment for participants to build a community with folks and staff who have lived knowledge of Black Culture and African American experience. This center offers individual and group counseling, peer support, and limited case management.

Karibu provides individualized, culturally specific Stabilization and Treatment Preparation (STP) services for Black/African American adults (18+) who are presently or at-risk of becoming criminal justice-involved and have behavioral health needs. This program has an outreach and supportive housing team which can support up to 30-35 participants in the community. The Karibu Community Care Team can provide clinical and case management, behavioral health assessment and care planning, individual and group therapy, service navigation and advocacy, and crisis planning/intervention among other services.

Old Town Clinic (OTC)

Like Blackburn Center, OTC offers physical and mental health services in a single location. OTC can offer sliding-scale services, including mental health counseling, medication management, and addiction/substance use treatment/recovery.

Old Town Recovery Center

Referrals for Old Town Recovery Center CORE or ICM Teams can be made by the client’s current provider.


The Old Town Recovery Center offers comprehensive and compassionate mental health services and outpatient treatment for substance use disorders. Their services include individual counseling, medication management, case and money management, in-home visits/outreach, and wraparound care. Their mental health professionals can do in- person, video, and phone appointments.

Since 2005, Puentes has offered culturally affirming and supportive environment for healing and recovery to Latine/Hispanic communities. Their bilingual and bicultural staff provide individual and group counseling, peer support, recovery groups, case management, outpatient addiction/substance use treatment, and more.

CCC’s Benefits and Entitlements Specialist Team (BEST) program helps particularly vulnerable individuals—those experiencing homelessness or deep poverty and living with severe disabilities that keep them from gaining employment—navigate the maze of the social security system. In order to access BEST services, intake with OTC will need to be completed.


The application process for social security benefits, Social Security Disability Insurance (SSDI) and/or Supplemental Security Income (SSI), can be daunting. BEST’s benefits specialists work with each client and use their expertise to maximize the chances of a successful application. They can coordinate psychological and medical exams; covered by BEST. BEST specialists will assist clients with initial application and appeals process.
